Washington, PA Washington, PA Full-time Full-time $42,000 a year $42,000 a year The Case Management Specialist is responsible for ensuring that individuals with a substance use disorder (SUD) have access to a continuum of care.
This is done by assessing the needs of the individual, determining the level of care (LOC) for treatment, developing a case management service plan; and when warranted, facilitating the individual's treatment.
Case managers develop a rapport with the individual serving as their advocate, assisting them throughout the provisions in their service plan and monitoring their progress while they are involved in the treatment system.
The Case Management Specialist will develop a rapport with each referring entity while providing direct case management services to each patient throughout the entire treatment experience.
This person will perform work in accordance with established regulations, policies, and procedures; but is expected to exercise initiative and independent judgment while performing the duties of his/her post.
The essential duties of the Case Management Specialist include, but are not limited to:
Preform LOC assessments at WDAC and in various community settings (ex:
jail, hospital, WDAC office) Ensures all patients are connected to the appropriate level of care and funding is secured for treatment episodes Regular, weekly contact with referral sources and treatment providers as needed Provides updated information from all programs and entities that interface with the agency Proactively interacts with each patient to meet their case management needs at the treatment and community level Required Knowledge, Skills and Abilities:
A minimum of a 4-year BA or BS in a Human Services related field is required The ability to work both independently and as part of a team Knowledge of principles and practices of persons in recovery A passion for service to others The ability to communicate effectively, both orally and in writing The ability to establish and maintain working relationships with representatives of local, state and federal agencies; health organizations and institutions; community agencies and organizations; and the general public Experience/exposure to various treatment facilities and community-based resources is highly recommended Must be able to pass a background check.
Having updated clearances is preferred but not required Partial work from home schedule (3 days in office, 2 work from home) after 90 day probationary period Yearly raises 10 paid holidays PLUS your birthday off Job Type:
