Develop and assure completion of monthly individual summaries.
Assess, evaluate and document individual behaviors and skills.
Assist in developing, implement, monitor and document goal plans and behavior modification

Instruct and model appropriate monitoring of individuals in areas including, but not limited

Coordinate and document individual care with family/guardian, Independent Supports
Coordinator, medical personnel, vocational placements, and monitor staff with the same.
Oversee and arrange for or transport individuals as necessary, to appointments, recreational

Qualifications:
Valid Pa Drivers License
18 years of age
3 year clean driving record
Associate's degree (A.A.) or equivalent from a two-year college in social work, psychology, special education, child development, or related field with emphasis on client management in residential settings, human development, or mental retardation
Minimum of 1 year direct care experience or related experience and/or training; or equivalent combination of education and experience.Estimated Salary: $20 to $28 per hour based on qualifications.
